Between-session accountability template

A simple template for capturing what a client commits to between sessions and following it up: the commitment, who owns it, the due date, evidence of progress, any blocker, and the next check-in. It structures follow-through; it is not a scorecard.

The accountability template

Copy it into your notes, or print it as a worksheet. Nothing here is stored.
BETWEEN-SESSION ACCOUNTABILITY
Client: ______ Session date: ______

Commitment 1
   What: ______
   Owner: ______ Due: ______
Why it matters (link to goal): ______
Evidence it is done (what I will see): ______
   Blocker / support needed: ______

Commitment 2
   What: ______ Owner: ______ Due: ______
   Evidence: ______ Blocker: ______

Next check-in: ______ (date / how)

When to use it

Fill it in the last few minutes of a session, together with the client, so the commitment is theirs and written down before they leave.

How to use it

Keep commitments few and concrete. Each needs an owner (usually the client), a due date, and a way you will both know it is done. Naming the likely blocker in advance is what separates a commitment from a wish. Set the next check-in so follow-through is visible before the next full session.

Example

What: have the delegation conversation with one report. Owner: client. Due: next Wednesday. Why: frees time for the strategy work that is the real goal. Evidence: the task is handed over and diarised. Blocker: guilt about "dumping"; support: rehearse the opening line. Next check-in: a two-line message on Thursday.

Mistakes to avoid

Vague commitments ("think about it more"). No due date. The coach owning the action. Only reviewing commitments at the next session, so a whole fortnight of drift is invisible. Treating a missed commitment as failure rather than data.

Adapt it

For habit work, replace single commitments with a cadence (e.g. three times this week) and track streaks. For team coaching, add "who else needs to know".

How this works inside Exantur

Exantur carries accountability items across sessions, lets clients update them in their own portal, and adds check-ins and habit-trackers so follow-through is visible between meetings, not just recalled at the next one. Optional; the template stands alone.

Frequently asked questions

How is this different from the accountability scorecard?
The scorecard rates how well your accountability system works. This template is the working document you fill with a client to capture and follow up commitments.
Who should own the commitment?
Almost always the client. Coach-owned actions quietly shift responsibility away from the person doing the changing.
Is anything stored here?
No. It is a template to copy, print or adapt; nothing you enter is saved.
